This form is for staff to report sexual misconduct concerns relating to any employees, volunteers or workers at the Trust. Once submitted, your report will be securely received by the People Advisory Team and reviewed to determine the most appropriate next steps. This may include informal resolution, support based interventions, a formal investigation, or referral into other relevant Trust processes (for example disciplinary and safeguarding procedures), depending on the nature and seriousness of the concern raised.
Your information will be handled sensitively and in confidence. It will only be shared where necessary to manage the concern appropriately, including with relevant managers or specialist teams. In some cases, information may need to be shared more widely where there are safeguarding considerations, risks to individuals, or legal or regulatory obligations requiring the Trust to take action.
Where you have provided contact details, a member of the People Advisory Team or an appointed case manager may contact you to seek further information, clarify details, or support next steps. This may include inviting you to attend meetings or provide additional evidence. You are expected to provide accurate and honest information to the best of your knowledge. You may request support or adjustments at any stage of the process.
You can choose to submit your report anonymously, however anonymous reporting may limit the Trust’s ability to fully investigate the concern or provide updates (where appropriate to do so) on the outcome.
Submitting this form does not automatically result in a formal investigation. All concerns are assessed on receipt to ensure they are managed in a fair, proportionate, and appropriate way.
